Summary : Mr Dinah was serving an indefinite term of imprisonment at Casuarina Prison. He died suddenly on 1 October 2018 in his cell.

Mr Dinah was known to have been suffering significant health issues, including poorly controlled diabetes and complex chronic heart disease. He was generally frail and unwell prior to his death, but at his request had been moved from the prison infirmary to the mainstream prison population.

On 1 October 2018 Mr Dinah appeared to be his usual self and advised after lunch that he wanted to have a sleep. He was put into his bed on the bottom bunk. When he was checked at 3.00 pm for afternoon muster he was found still lying in his bunk but he could not be roused. Staff were called to his cell immediately and CPR was commenced. A defibrillator was used and one shock was delivered. St John Ambulance was called and arrived at 3.23 pm. After he was assessed, a decision was made for all resuscitation efforts to be ceased.

The Coroner found the cause of death was atherosclerotic heart disease in a man with diabetes mellitus.

The Coroner concluded that Mr Dinah’s medical treatment and care while in custody was of a high standard.
