Coroner's Court of Western Australia

Inquest into the Death of Tom Foski

Delivered on 11 April 2014 at Perth.

Finding of Coroner King.

I recommend that case managers of long term mental health inpatients whose finances are controlled by the Public Trustee’s Office consult with the Public Trustee with a view to implementing a process of exchange of information in order to use funds held on behalf of the patients to improve the patients’ quality of life.

The deceased was a 51 year old male who died on 6 May 2012 and who was an involuntary patient at Graylands Hospital.

The inquest was held together with the deaths of nine other persons who had been held in care as involuntary patients at Graylands Hospital immediately before they died.

The Coroner found the deceased died 6 May 2012 at Sir Charles Gairdner Hospital from congestive cardiac failure in association with ischaemic and hypertensive heart disease. The manner of death was natural causes.
