Inquest into the Death of Daniel LAHENGKING

Summary : Daniel Lahengking was a 66 year old man at the time of his death on 15 August 2009 at Royal Perth Hospital from multiple organ failure associated with the combined effects of complications of methotrexate toxicity, vasculitis and atherosclerotic cardiovascular disease.

Coroner King conducted an inquest hearing at the Esperance Court on 28 to 29 May 2014 and investigated what role, if any, methotrexate played in the death and the respective responsibilities of prescribing doctors and dispensing pharmacists when the type or dosage of medications on a prescription appears to the pharmacist to be wrong.

Coroner King made an open finding as to how the death occurred.

Coroner King found that the quality of medical care provided to the deceased was well below the standard reasonably expected and under s50 of the Coroners Act 1996, Coroner King referred the matter to the Australian Health Practitioner Regulatory Agency.
