Summary : At the time of his death of Mr Jacovic was a sentenced prisoner at Acacia Prison. Mr Jacovic died from ischaemic heart disease on 22 June 2018. He was 63 years old.

Mr Jacovic had been incarcerated for a lengthy period and during that time he had received considerable medical input for his physical and mental health disorders. On 12 June 2018 Mr Jacovic suffered heart failure and a heart attack. He was transferred to hospital for treatment. Investigations revealed severe impairment of his heart’s function and extensive coronary artery disease that was not amendable to re-opening with stenting or further bypass grafting. Medical advice was to proceed with medical management of his condition as he was not suitable for surgical intervention.

On 21 June 2018 Mr Jacovic was transferred back to prison with a long term plan for cardiology follow-up as an outpatient and ongoing medical management of his heart failure with medication and fluid restriction. He was housed in the medical ward of Acacia Prison, where he appeared initially well, but the following day he suffered a cardiac arrest. He was found unresponsive and resuscitation attempts were unsuccessful.

The Coroner was satisfied that Mr Jacovic was given a high level of medical care and supervision.
