Summary

Eden Ryan Zanetti died on 21 November 2024 at the age of 40 years old while subject to a Community Treatment Order (CTO) under the Mental Health Act 2014 (WA).

Eden was diagnosed with treatment resistant schizoaffective disorder, and had an extensive mental health history. He was admitted to St John of God Midland Hospital on 8 October 2024 following a relapse of his schizoaffective disorder, due to his non-compliance with treatment in the community.

He remained in hospital until 14 November 2024, when he was discharged on a CTO, under the management of the Bentley Adult Community Mental Health Service.

On 21 November 2024, Eden went to Boya Quarry and ended his life by jumping from the top of a cliff at the Quarry.

The coroner found that the supervision, treatment and care provided to Eden by St John of God Midland Hospital, and by Bentley Adult Community Mental Health Service, was of an appropriate standard.
