Summary : The deceased was 25 years old at the time of her death.

On the evening of 21 January 2015 the deceased used methadone intravenously with her friend at his mother’s house. He then drove her home in the early hours of 22 January 2015, and that day the deceased went to work.

On the evening of 22 January 2015, the deceased returned to her friend’s home and again used methadone. After the deceased’s second injection of methadone that evening, she went to sleep.

The deceased was still asleep or comatose when her friend woke on the morning of 23 January 2015. He placed her in the recovery position and placed part of a wafer of Suboxone under her tongue in an attempt to counteract the effect of the methadone. He then left the house after asking a house guest to check on the deceased from time to time. About an hour later, the house guest found the deceased to be unresponsive. Ambulance officers attended, but the deceased showed no signs of life and she could not be revived.

A forensic pathologist concluded that the deceased’s death was due to complications of opioid toxicity.

The Coroner found that death occurred by way of accident and made no recommendations.
