On the basis it would be contrary to the public interest, the Court makes an order under s 49(1)(b) of the Coroners Act 1996 (WA) that there be no reporting or publication of the name of any prisoner (other than the deceased) housed at Acacia Prison on 11 November 2024.

Summary

Mr Mason Shahzad was a 40-year-old sentenced prisoner when he died on 11 November 2024 at Acacia Prison after hanging himself in his cell. Mr Shahzad died from ligature compression of the neck and the Coroner found his death occurred by way of suicide.

The Coroner determined that Mr Shahzad received adequate treatment, supervision and care while in custody. Issues concerning delays in transfer of Mr Shahzad’s personal property between prisons were considered, including improvements made by the operators of Acacia Prison since Mr Shahzad’s death.
