Summary : The deceased was last seen in the early hours of Wednesday 19 September 1979 on board a bulk carrier, the Tambo River, which was anchored off Port Hedland waiting clearance for the port.

At 11.00 pm on 18 September 1979 the deceased spoke with the Master of the vessel who had the impression the deceased appeared to be affected by alcohol and had difficulty comprehending instructions. The deceased was later seen drinking in the recreation room of the vessel and he left the recreation room to go to his cabin to sleep.

The Tambo River had been anchored at the Port Hedland Anchorage and it departed for the Port Hedland docks shortly after 6.30 am on Wednesday 19 September 1979. A crew member went in search of the deceased and failed to locate him in his cabin. An immediate search was conducted of the ship before report the deceased as missing. Police were notified at 9.50 am on 19 September 1979 that the deceased was missing. Despite searches undertaken by police the deceased has never been located.

The Coroner was satisfied the deceased fell overboard while intoxicated from the Tambo River on 19 September 1979 at sea in the vicinity of Port Hedland Anchorage about 7 nautical miles southwest of Port Hedland.
